Registration Information
***Please read
this important information
before completing the Registration
Form***
- A parent or guardian must complete the application form in printing.
- Please ensure that all areas are filled. Please also provide additional
information for medical concerns if necessary. In particular, medication
that your son/daughter may require and will be taking during the camp
must be listed.
- Please use one application form per camper.
- Payment must accompany your application form.
- Refund/N.S.F. Policy - Please note that cancellations are subject to
a $15 administrative fee if cancellation is received 2 weeks prior to
the start date. Cancellations after the 2 week cutoff will be refunded
at 50%. There will be no discounts for days not attended. N.S.F. cheques
are assessed a $28 administration fee. There is no refund once camp
begins, but suitable substituions are welcome.
- Only full session enrollments are acceptable.
- Players are eligible as long as they are enrolled in high school in
2010, either beginning grade 9 in September 2010, or finishing high
school in June 2010. Elite Grade 8 players will also be
considered.

- Payment Options
- After you have completed the Registration Form, click the 'submit
button' at the bottom of the form and you will be directed
to either:
- Pay by VISA - $195.00 - you will be re-directed
to the CMR Governance Consulting website for payment
- Please note: Charge will appear on Visa statement
as "CMR Gov Cons"
- Pay by cheque - $195.00 - Please make payable to Joe
Raso Basketball Camp and mail completed Registration Form and cheque

Team Discounts
Available - Group discount of $10.00 per player for
schools or teams that register 5 or more athletes at the same
time.
