Registration Information
***Please read
this important information before completing the Registration
Form***
- A parent or guardian must complete the application form in printing.
- Please ensure that all areas are filled. Please attach additional
information for medical concerns if necessary. In particular, medication
that your son/daughter may require and will be taking during the camp
must be listed.
- Please use one application form per son/daughter.
- Payment must accompany your application form.
- Refund/N.S.F. Policy - Please note that cancellations are subject to a $15 administrative fee if cancellation is received 2 weeks prior to the start date. Cancellations after the 2 week cutoff will be refunded at 50%. There will be no discounts for days not attended. N.S.F. cheques are assessed a $28 administration fee.
- Only full session enrollments are acceptable.
- Players are eligible as long as they are
enrolled in high school in 2008, either beginning grade 9 in
September 2008, or finishing high school in June 2008. Elite Grade 8 players will also be considered.

Team Discounts
Available - Group discount of $10.00 per player for
schools or teams that register 5 or more athletes at the same
time.
