Registration Information

Please read this important information
before completing the Registration Form
  1. A parent or guardian must complete the application form in printing.

  2. Please ensure that all areas are filled. Please also provide additional information for medical concerns if necessary. In particular, medication that your son/daughter may require and will be taking during the camp must be listed.

  3. Please use one application form per camper.

  4. Payment must accompany your registration form.

  5. Refund/N.S.F. Policy - Please note that cancellations are subject to a $15 administrative fee if cancellation is received 2 weeks prior to the start date. Cancellations after the 2 week cutoff will be refunded at 50%. There will be no discounts for days not attended. N.S.F. cheques are assessed a $28 administration fee. There is no refund once camp begins, but suitable substituions are welcome.

  6. Only full session enrollments are acceptable.

  7. High School Camp: Players are eligible as long as they are enrolled in high school in 2011, either beginning grade 9 in September 2011, or finishing high school in June 2011.  Elite Grade 8 players will also be considered.

    3. Payment
      After you have completed the Registration Form, click the 'submit button' at the bottom of the form

      Pay by cheque - payable to Joe Raso Basketball Camp:
      Day Camp - $185.00 taxes included
      High Performance High School Camp - $125.00 taxes included

      Please make cheque payable to Joe Raso Basketball Camp and note on the cheque the name of the camper(s). Please mail cheque to:

      NOTE: Cheques can only be mailed to the Upper James address because it is a post office box. If you wish to deliver your cheque in person, please call 905-541-5216. Thank you.
